Learning Objectives: 

  • Understanding Problem Scoping and Opportunity Identification:
       - Participants will learn how to effectively scope the problem of managing type 2 diabetes in high-risk populations and identify potential areas where a "Food as Medicine" approach can have meaningful impacts. This includes exploring the social determinants of health-related to nutrition insecurity and analyzing data to pinpoint specific needs and opportunities for intervention in targeted populations.

 

  • Aligning and Designing a Targeted Program:
       - Attendees will gain insights into designing and implementing a technology-driven "Food as Medicine" program tailored for high-risk members with type 2 diabetes. This will cover strategies for program alignment among stakeholders, setting clear objectives, and using technology to provide nutrition support. It will also include best practices for engaging and collaborating with key partners like NourishedRx to maximize reach and effectiveness.

 

  •  Measuring Outcomes and Impact:
       - Participants will explore methodologies for measuring the impact of the "Food as Medicine" program on clinical outcomes, nutrition security, and self-reported lifestyle behaviors. This will involve a detailed examination of data collection, analysis techniques, and outcome metrics to evaluate program success and derive insights. The session will offer guidance on using these evaluations to refine programs and inform future initiatives effectively
