Event Overview
Achieve better clinical outcomes.
Module 6 will focus on Health Outcomes. This module centers on the development and implementation of programs aimed at achieving better clinical outcomes, a key factor in Star measures. Substantial emphasis on the Health Outcomes Survey (HOS) measures, particularly those related to Medical and Behavioral Health outcomes, acknowledging their growing importance in Star Ratings. Delve into other outcome-based initiatives like reducing readmission rates, and explore related topics such as overall health status, the effectiveness of Health Risk Assessments (HRAs), and medication use. Importantly, this module will include a discussion on Special Needs Plans, highlighting the integration of Star measures into care management and the Model of Care, underscoring the importance of tailored approaches in meeting the unique needs of these populations.
This module is designed to provide participants with the insights and tools necessary to effectively enhance health outcomes, thereby directly contributing to improved Star Ratings for their Medicare Advantage Plans.
In-Session: Planning the work
This workshop will focus on Collaboration and Activation. Through large group lecture and small groupwork, augmented with working sessions beside your cohort peers, we'll help you walk away with the following accomplishments:
- Innovation Session: Addressing HOS measures in Medical and Behavioral Health.
- Leadership Discussion: Strategies for outcomes-based initiatives such as reducing readmission rates.
- Barrier Buster: Tailoring approaches for Special Needs Plans and integrating stars into care management.
Homework: Work the plan
The work continues when you return home. We encourage you to have a debrief meeting already scheduled on your Stars team members' calendars so you can dive right into the action items you've framed. Look forward to a follow-up meeting with your Accelerator cohort in about 2 weeks, when you can share your progress and get help with any barriers.

This meeting is limited to Health Plan Alliance members participating in the 2024 MA Stars Accelerator and invited guests. Each health plan Accelerator champion should ensure their organization sends at least one individual. You may send up to two delegates, if you wish.
